I'm trying to remember. There was at least one of the four occasions, and possibly two where he just didn't show up for work and didn't call in, so you could say he quit, technically. He was fired the one time for long lunches and coming back stoned. I can't remember the other firing. I know the last time they hired him, when I objected, he only lasted a few weeks before being gone again, so I felt vindicated in my criticism of the decision to re-hire him.
